refactor `ParamEnv::empty(Reveal)` into two distinct methods
- `ParamEnv::empty()` -- does not reveal all, good for typeck - `ParamEnv::reveal_all()` -- does, good for trans - `param_env.with_reveal_all()` -- converts an existing parameter environment
