Remove leading newlines; replace lines containing only whitespace with empty lines; replace multiple trailing newlines with a single newline; remove trailing whitespace in lines

The previous wording was confusing. While would we need to go through
the whole list just to find the first code point? `chars()` being an
iterator, we only need to walk from the beginning of the list.

This adds strictly more information to the source files and reduces the
need for customized tooling to render the book.
(While this should not change the output of _rustbook_, it is very
useful when rendering the sources with external tools like Pandoc.)

Newcomers to Rust need to learn the distinctinion between `&str` and
`String`, so additonally having `string` in an example risks confusion.

formatting in "The Rust Programming Language" as discussed in #19823.
- Convert bold text to italicized textcwhen referring to terminology.
- Convert single-quoted text to italicized or double-quoted text,
depending on context.
- Use double quotes only in the case of scare quotes or quotations.
