This updates the reference which is now using a new mdbook plugin. This
requires a little extra work than a normal book because the plugin uses
`rustdoc` to generate links to the standard library. It also ensures
that the submodule is available for *any* command that uses rustbook,
since it is now part of the rustbook workspace.

`rust-lang/book` recently added two mdBook preprocessors. Enable
`rustbook` to use those preprocessors for books where they are requested
by the `book.toml` by adding the preprocessors as path dependencies, and
ignoring them where they are not requested, i.e. by all the books other
than TRPL at present.

rfc3052 followup: Remove authors field from Cargo manifests
Since RFC 3052 soft deprecated the authors field, hiding it from
crates.io, docs.rs, and making Cargo not add it by default, and it is
not generally up to date/useful information for contributors, we may as well
remove it from crates in this repo.

As rustbook now depends transitively on openssl, it needs access to the
rustc-workspace-hack/all-static feature to pick up openssl-sys/vendored.
This fixes the rust build with `all-static = true` on systems where
openssl is not installed (e.g. when cross-compiling).
|

This updates the last of the books using mdbook 0.1, finally
removing it from the build.
